چکیده

Introduction In intubated acute respiratory failure patients, inhomogeneity of ventilation-perfusion matching (i.e., presence of over-ventilated under-perfused lung regions) might determine extreme pH shifts and increase the risk of developing ventilator-induced injury. Positive end-expiratory pressure (PEEP) redistributes tidal ventilation towards more perfused dependent lung regions, potentially improving homogeneity of ventilation-perfusion matching.

Perfusion of the interventricular septum during ventilation with positive end-expiratory pressure.

OBJECTIVE To determine whether regional hypoperfusion of the interventricular septum occurs during ventilation with positive end-expiratory pressure. DESIGN Animal study. ANIMALS Anesthetized, closed chest dogs (n = 8). INTERVENTIONS Induction of experimental adult respiratory distress syndrome (ARDS) and then ventilation with 10, 15, and 20 cm H2O of positive end-expiratory pressure. M...

Effects of pressure-support ventilation with different levels of positive end-expiratory in a mild model of acute respiratory distress syndrome

Introduction Pressure-support ventilation improves lung mechanics, blood gas exchange, hemodynamics, and work of breathing (WOB) in mild acute respiratory distress syndrome (ARDS) [1,2]. Nevertheless, those beneficial effects could be dependent of positive end-expiratory pressure (PEEP) applied during mechanical ventilation. So far, no study has compared pressure-support ventilation (PSV) with ...

Effect of body position on ventilation distribution during PEEP titration in a porcine model of acute lung injury using advanced respiratory monitoring and electrical impedance tomography

BACKGROUND Lung failure after acute lung injury remains a challenge in different clinical settings. Various interventions for restoration of gas exchange have been investigated. Recruitment of collapsed alveoli by positive end expiratory pressure (PEEP) titration and optimization of ventilation-perfusion ratio by prone positioning have been extensively described in animal and clinical trials. T...

Respiratory controversies in the critical care setting. Does airway pressure release ventilation offer important new advantages in mechanical ventilator support?

Airway pressure-release ventilation (APRV) is a mechanical ventilation strategy that is usually time-triggered but can be patient-triggered, pressure-limited, and time-cycled. APRV provides 2 levels of airway pressure (P(high) and P(low)) during 2 time periods (T(high) and T(low)), both set by the clinician. APRV usually involves a long T(high) and a short T(low). APRV uses an active exhalation...

Comparison of the effects of PEEP levels on respiratory mechanics and elimination of volatile anesthetic agents in patients undergoing laparoscopic cholecystectomy; a prospective, randomized, clinical trial.

BACKGROUND/AIM In laparoscopic procedures, intraabdominal carbon dioxide (CO2) insufflation can cause decreased compliance, increased airway resistance, and impaired ventilation-perfusion ratios. We aimed to investigate the effects of intraoperative positive end-expiratory pressure (PEEP) treatment on respiratory dynamics and elimination time of volatile anesthetic agents. MATERIALS AND METHO...
